Data and operation
The form asks for surname, first name, telephone number and email address. All fields are required to prepare the request. Do not enter any medical information. After validation, the website opens the user’s email application with a pre-completed message. The website does not send the message itself and does not record these details in a database.
Purposes, legal basis and recipients
The details identify the request, allow the relevant secretariat to contact the person and, where appropriate, organise an appointment. Processing is based on pre-contractual measures taken at the person’s request. Recipients are Dr Clair-Florent Schmitt-Bernard and authorised staff of either Clinique de la Vision Montpellier or Odysseum Ophtalmologie, depending on the selected request.
Retention
If a request does not lead to care, the email and its data are deleted no later than one month after the last exchange. If a consultation takes place and information is incorporated into the patient record, it is kept with that record for twenty years from the last episode of care, subject to statutory extensions or suspensions.
Security and absence of a website database
The public website does not store the form data or maintain a patient database. Connections are encrypted using HTTPS. Once the user sends the prepared email, its security and retention also depend on the user’s and recipient’s email services. Medical information must not be entered in the appointment form.
Hosting data, cookies and external services
Cloudflare may process technical data required to deliver and secure the website, including IP address, request date and time, page address and browser characteristics. The publisher has not installed advertising or audience-measurement cookies. Cloudflare may use the strictly necessary __cf_bm security cookie, which it states expires after thirty minutes of inactivity.
Images and logos are served from this website. Instagram, LinkedIn, practice locations and scientific publications are contacted only after the user clicks the corresponding external link.
YouTube videos are not loaded automatically. Information is shown before each video, and the connection to YouTube’s servers occurs only after the user chooses to load the video.
Your rights
Subject to legal conditions, you may request access, rectification, erasure, restriction, objection and data portability. Proof of identity may be requested only where there is reasonable doubt about the requester’s identity. You may also lodge a complaint with the French data-protection authority (CNIL).
